Dental Insurance

Employees may select from two traditional dental insurance plans: a high option plan and a low option plan. Permanent employees who work at least 20 hours per week for nine or more months per year are eligible to enroll. Premiums are paid on a pre-tax basis.

Traditional Plans allow employees to see any dentist they choose. They provide coverage for preventive and diagnostic services, Basic Services and Major Services after satisfying the yearly deductible; thereafter, benefits are paid based on a percentage of the usual, customary, and reasonable charge. Under these plans the Dentist may or may not file claims for you.

The High Option covers Preventive and Diagnostic Services at 100% of their cost with no deductible costs. Most Basic Services are covered for 80%, and Major Services and Orthodontics at 50% of their cost after meeting a $25 annual deductible. The employee pays a higher monthly premium for this option.

The Low Option covers Preventive and Diagnostic Procedures at 100% of their cost with no deductible costs. Most Basic Services are covered at 50% of their cost with an annual deductible of $50. The low option offers no coverage for Major Services or Orthodontics. Employees pay a lower monthly premium for this option.
